Important Notice for All Members, Trial Swimmers and Guests

To complete your registration with the Oakville Masters Swim Club, you must submit the "RELEASE OF LIABILITY, WAIVER OF CLAIMS AND INDEMNITY AGREEMENT" and "CONSENT" form. 

Why This Is Required:

• Safety: Ensures all members are aware of and accept the risks associated with swimming activities.
• Legal Compliance: Meets legal requirements to protect both members and the club.
• Consent: Confirms members agree to the terms and conditions of participation.

How to Complete Your Registration:

2. Read and Agree: Complete all required sections of the form.

3. Submit: Completed forms are logged for the club's administration team.
